• Visit our pre-registration Web page and click on “Sign-Up Now”. Provide your e-mail address and select a password.



• Re-visit the pre-registration Web page and log in. Once you have logged in, click on the “Procedure Appointment Request” button under Appointments.



• Fill out the request form and select “Screening mammogram” from the procedure menu. You do not need to list a referring physician, but you will need to list a doctor to receive the test results. Your family doctor or OB-GYN is fine.



• After you have submitted your request, the scheduling department will e-mail you with possible appointment dates and times. You must respond to their e-mail to confirm your appointment.
